Solute Service requests this information because we need to be sure we have considered all your health needs about the duties you are required to perform, so we do not needlessly risk your health in duties you will perform as a cleaner.
You are solely responsible for informing solute service managers on this form page and other forms of contact issued to you, of your health concerns during the duty of cleaning.
At times we may request you work with another cleaner who does not have the health to manage a particular duty, we would inform you of your duties. We require you to inform our manager and administrators at solute service.co of any personal health concerns, you may have to complete the health and wellness form/questionnaire during the active duty of cleaning if you have any health problems that you think your health would be at risk in a duty please inform us as soon as possible.
You will be asked to proof of your health concern, though this is not mandatory.
You must agree to follow our request regarding your health and well being questionnaire. We may ask you to not proceed in a duty/skill, and give this duty to another cleaner or ask you to stop cleaning if you cannot produce a 'fit to work' letter from your doctor that you are willing to share/view by solute service management.
